Retatrutide
Triple receptor agonist | Metabolic Health • Weight • Emerging Therapeutics
4V Take
A clearer starting point.
Retatrutide has produced very large average weight loss in late-stage trials. It is still investigational and not FDA-approved, and we need more long-term safety and outcome data. We also should not assume it beats tirzepatide until they are directly compared.

4V Take
Retatrutide is an investigational once-weekly drug that activates three receptors: GIP, GLP-1, and glucagon. That is why it is called a triple receptor agonist. It is sometimes nicknamed ‘GLP-3’ online, but there is no GLP-3 receptor involved.

4V Take
Retatrutide has produced very large average weight reductions in its clinical program. In sponsor-reported Phase III TRIUMPH-1 results, the 12 mg group averaged about 28.3% weight loss at 80 weeks under the efficacy estimand, with a prespecified extension reporting about 30.3% at 104 weeks in a higher-BMI subgroup. Those are trial averages, not predictions for an individual, and the obesity Phase III results should remain clearly labeled as sponsor/meeting-reported until the full peer-reviewed publication is available.

4V Take
Yes. In a peer-reviewed Phase III trial in adults with type 2 diabetes inadequately controlled with diet and exercise, retatrutide lowered A1C and produced substantial weight loss compared with placebo. The exact results belong to that studied population and regimen. It adds strong human evidence that retatrutide’s metabolic effects extend beyond weight alone.

4V Take
No. As of August 22, 2026, retatrutide remains investigational and is not approved by a regulatory agency. That status does not erase the positive Phase III evidence, and positive trials do not themselves equal approval. Regulatory status and scientific findings are related questions, but they are not the same question.

4V Take
The most common adverse effects reported in retatrutide trials have been gastrointestinal, including nausea, diarrhea, constipation, and vomiting. Phase III reports have also described dose-related dysesthesia, meaning unusual or unpleasant skin sensations such as tingling or altered sensitivity. Trial rates describe study populations and should not be converted into a personal risk estimate.

4V Take
Retatrutide has produced striking weight-loss results, but separate-trial percentages do not prove that it is superior to tirzepatide. The trials differ in populations, duration, analysis methods, and protocols, and 4V has not identified a completed direct head-to-head trial supporting a simple superiority claim. We can compare the evidence, but we should not pretend a cross-trial scoreboard is the same as a direct comparison.

Evidence Boundaries
What are researchers still working out?
Study settings, measures, and limitations matter. A mechanism does not establish an outcome, animal or laboratory findings do not establish a human benefit, and a small study does not create consensus. Open each selected source for the approved record behind its finding.
Regulatory Context
Context, not a shortcut.
This topic’s selected public record includes regulatory or other context sources. They can help explain the setting in which research is discussed, but regulatory status does not by itself prove or disprove scientific value. Open the original source for its exact context.
