What this notice covers
This notice describes the information handled through the current 4V Elevated site and its public forms. It is an operational notice about current practices, not legal advice. Please do not send information that is not needed for your request.
Interest and update requests
When you ask for 4V, Cuatro, Shop, Research, or Coaching / Education updates, we record your email address, optional first name, selected update types, source and campaign information when provided, consent timestamp, privacy-policy version, and record status. This information supports the update request you selected and is kept separate from safety reporting.
Support and product-quality requests
When you submit ordinary support, product-quality, packaging, lot, or documentation requests, we record the contact information and concise request you provide. This can include order, product, and lot references; request category; quality issue type; workflow status; and private team notes needed to review the case. A submitted request does not automatically change a product, lot, release, recall, or COA status.
Possible safety reports
The separate possible safety-report path records the reporter contact information, supplied product, lot, and order references, and the brief report submitted through that form. Depending on what a reporter provides, this may include safety or adverse-event details that could be health-related. Safety records are kept separate from interest and marketing records and are used for private human review; 4V does not diagnose, provide treatment, or offer medical advice through this path.
Site, account, and service information
The site uses hosting and delivery infrastructure, an application and database platform, and approved asset delivery services to operate its pages and forms. If site analytics is configured, PostHog may process site-use, device, and cookie or local-storage information to help understand site operation. Account functionality, where used, processes the account information needed for sign-in and access. Service providers may process information on 4V’s behalf to provide these infrastructure functions.
Retention and human review
4V keeps records for as long as reasonably needed for the request, operations, safety review, fraud prevention, accounting, or other applicable obligations. The current system uses retention categories and review holds rather than automatic deletion timers. Safety reports and product-quality records are not automatically deleted, and a privacy request does not automatically delete, export, or change any record.
Privacy requests
You may use the privacy request form to request access, correction, deletion, a portable copy, or an appeal or follow-up. The form collects only your name, email, request type, and concise request details. Each request is recorded for verification and human review before any action is considered.
Revision record
Version 2026-08-20 established the operational site notice. Version 2026-08-24, effective August 24, 2026, adds current data-category, privacy-request, retention, and safety-record governance information. Earlier consent timestamps and policy-version values remain on their original records.
